How will changing my last name affect my Verizon account and e-prescribing?

The provider will need to make sure to change their name with NPPES, in their application and NewCrop will also verify that the name has been updated with SureScripts.

Once the last name update is verified in these locations, please contact Stratus EMR support, who will email NewCrop with the following information. The username can remain the same and any username changes need to be noted so that once the updates are made, the provider knows the new username. (The provider will use the new username only after NewCrop has verified the username and profile have been updated.) NewCrop will email this information to Verizon and NewCrop will contact you once the information has been updated. The following information will be needed:

If the provider is updating an email address along with the last name, the provider will need to add the email address by logging into Verizon, then going to:

My Profile > View My Profile > Edit My Profile

then clicking the "+" symbol to the right of the existing primary email address, and entering the address then saving. The provider then needs to request a new certificate. When requesting a new certificate, there will be a dropdown at the password prompt to select the email that they want to use. The new certificate request will be emailed and they will sign the new certificate.

Please notify Stratus EMR when this step is completed; we will contact NewCrop to inform them. (If the provider is keeping the same email address, skip this step.) NewCrop will notify you once all information has been successfully updated. If a new username was selected, the provider will then use the new username upon notification.
